Care Facilitator – At Home Services
Utilise your experience in Case Management to support our clients living at home in the Northern Sydney region.

As a Care Facilitator servicing clients across, the Northern Sydney region you will assist clients and their families to engage in services, enabling them to remain living in their home.
You will empower our clients through individualised care planning and goal-setting focused on our well-living approach.

What do we need from you?
You will need to be comfortable working on your own in the community, have excellent verbal communication skills and have an empathetic, patient and caring nature.
This role is a full-time position working from home Monday to Friday, with approximately 60% of the role visiting new and existing clients.
This role does require you to have:
You will be supplied with a mobile phone, laptop, computer monitor and printer.
