About your new role 

As a Registered Nurse with BaptistCare Home Care, you will be supporting clients in Albany and surrounding areas and will play a vital role in providing high-quality care to clients living at home.

You will ensure care aligns with aged care quality standards while promoting independence, safety, and an improved quality of life.

Reporting to the Senior Nurse Consultant, your key responsibilities will be:  

  • Delivering Clinical Care 

  • Conducting Comprehensive Assessments and regular reviews   

  • Care Planning and Coordination  

  • Advocating for Clients  

  • Providing Health Education 

  • Monitoring Health and Reporting Outcomes 

  • Networking and Promoting BaptistCare Services   

Working hours 

This is a permanent part-time position for 45.60 hours per fortnight, equivalent to 7.6 hours per day, over three days per week.  
 
You will be operating independently in the community travelling to visit clients regularly and working from the local home care office in Albany.
 
There is a wide travel radius, you must be willing to travel to other Great Southern areas including Denmark, Mount Barker, Cranbrook, Kojonup and Katanning as needed.  Travel will be reimbursed as per the BaptistCare NSW and ACT Aged Care Enterprise Agreement.  
 

What do we need from you? 

You will have an unrestricted Australian Driver Licence and a reliable vehicle with Third Party Property Damage Insurance.

You will be a strong communicator with resilience and patience. You will be a proficient user of smartphone technology, apps and email.

You must have: 

  • Registered Nurse qualifications with current AHPRA registration  

  • Minimal 12 months clinical experience in acute or aged care sector  

  • Proven experience working within budget and service delivery parameters
 
 

  • Sound clinical decision making and problem-solving skills  

  • Demonstrated ability to work independently and as part of a team
