About the Role

The Wellness Coordinator plays a critical role in supporting residents to live independently, safely and with purpose. This position focuses on effective assessment, planning and coordination of individualised services that enhance residents’ wellbeing, social connection and quality of life.

Working closely with residents, families, carers, Village Management and external service providers, the Wellness Coordinator ensures residents are informed, supported and connected to appropriate services that align with their goals, strengths and evolving needs.

This role is based at Yarra Rosa Retirement Village, Red Hill, ACT. It is a part time position Monday to Wednesday 7.5 hours per day.


About the Team

You will be part of a collaborative Retirement Living team that is committed to resident-centred care, proactive wellbeing support and continuous improvement. The team works closely with Home Care, community services and external agencies to ensure residents receive seamless, culturally appropriate and timely support.

About You

You are a compassionate and organised professional who thrives on building relationships and supporting people to achieve meaningful outcomes. You bring a strong understanding of service coordination, resident engagement and wellbeing in a retirement living, aged care or community services environment.

Key Responsibilities

Your responsibilities will include, but are not limited to:

  • Assessing residents’ individual support needs, goals, strengths and vulnerabilities
  • Developing and coordinating personalised support plans
  • Promoting awareness and utilisation of available services, including Home Care
  • Referring residents to appropriate internal and external services as required
  • Supporting residents’ self-management and relationships with carers and family
  • Liaising with government agencies, service providers and community organisations
  • Maintaining accurate case notes, documentation and records
  • Monitoring and communicating changes in residents’ circumstances to the Village Manager
  • Ensuring services are delivered in a culturally appropriate manner, aligned to BaptistCare’s shared values and best practice principles
  • Contributing to a safe, inclusive and engaging environment that promotes participation and wellbeing
  • Complying with all BaptistCare policies, procedures, legislative requirements and WHS obligations
  • Participating in continuous improvement, professional development and team activities

Key Requirements

Essential Qualifications and Experience

  • Minimum 3 years’ experience in Retirement Living, Residential Care, Community Services, Home Care or a related discipline
  • Demonstrated experience providing individualised support and coordination services
  • Current Driver’s Licence

Essential Competencies

  • Strong negotiation and stakeholder engagement skills
  • Competence in assessment, service planning and documentation
  • Ability to work collaboratively within a cross-functional team
  • Sound knowledge of Work Health and Safety practices
  • Intermediate to advanced MS Office and Outlook skills
  • Strong written and verbal communication skills
  • Empathy, patience and resilience when working with residents
  • Flexibility to work across locations as required
  • Willingness to participate in ongoing education and professional development
  • Alignment with BaptistCare’s purpose, values and Christian identity

Desirable

  • Allied Health qualifications or relevant experience (e.g. Physiotherapy, Podiatry)
  • Experience working in a medical or clinical practice environment
