About the Role
We are seeking a compassionate and recovery-focused Mindset Practitioner to join our team and provide person-centred psychosocial support to people experiencing mental ill-health.
Working primarily in community and outreach settings, you will work alongside participants to build on their strengths, strengthen wellbeing, develop independence and work towards the goals that matter most to them.
You’ll have a high level of professional autonomy, supported by practice leadership from a Senior Practitioner and operational supervision from a Team Leader.
What you'll be doing
- Deliver individualised, recovery-focused psychosocial support.
- Work collaboratively with participants to identify goals, strengths and priorities.
- Support participants to build skills, confidence, independence and community connections.
- Provide flexible outreach support across home and community settings.
- Apply recovery-oriented, strengths-based and trauma-informed approaches.
- Promote participant choice, voice and self-determination.
- Build respectful relationships with participants, families, carers and support networks.
- Work collaboratively with mental health clinicians, health services and community organisations.
- Manage an allocated caseload, balancing complexity, risk and competing priorities.
- Identify, assess and appropriately escalate risks relating to participant safety and wellbeing.
- Maintain high-quality case notes, plans and reports in line with organisational and legislative requirements.
- Participate in supervision, reflective practice, professional development and continuous improvement.
About you
You are someone who builds genuine relationships, sees the person beyond their mental health challenges and is confident working independently in the community.
You will have:
- Demonstrated understanding and application of recovery-oriented, person-centred and strengths-based practice.
- Experience engaging people experiencing mental ill-health and providing community or outreach-based psychosocial support.
- The ability to assess and respond to risk and exercise sound professional judgement.
- Strong communication and collaboration skills.
- A commitment to culturally safe, inclusive and respectful practice.
- The ability to work autonomously while contributing positively to a team.
