About the Role:
Based at our 85-bed facility in Albany, this role focuses on creating and delivering a personalised, meaningful lifestyle experience for each resident. You will gather life history profiles and work closely with residents, families, and the multidisciplinary care team to develop individualised lifestyle plans that reflect each resident’s cultural, spiritual, emotional, and personal interests.
Working alongside the therapy and OT teams, you will help design and coordinate tailored group and individual programs, promote participation, and ensure residents are informed about upcoming activities. You’ll support and guide Therapy and Leisure Partners in adapting programs to resident needs, help plan special events and celebrations, and build community connections to enrich the lifestyle offering.
This is a dynamic role that blends planning, creativity, and collaboration, with the goal of enhancing resident wellbeing through personalised and culturally inclusive engagement.

About You:
You are a dynamic individual with a positive, “can-do” attitude who builds strong relationships with others. While a formal qualification in Health, Aged Care, Allied Health Assistance, or Community Services is preferred, it’s not essential. You have a proven ability to plan, implement, and evaluate innovative care programs, particularly for those with dementia.
With effective communication skills and the ability to manage a busy workload, you can adapt to changing priorities with ease. You work collaboratively in a team environment and are dedicated to continuously developing your knowledge and skills. Your IT proficiency at an intermediate level helps you manage tasks efficiently, and you are committed to a customer-centric approach in all that you do.
You bring experience as a Therapy Assistant, Lifestyle Coordinator, or Diversional Therapist in aged care and have a solid understanding of WH&S legislation and practice. Your ability to lead a small team and respond flexibly to the needs of the residents makes you a perfect fit for this role.
