About the role:

As the End-of-Life Care Coordinator, you will be responsible for the development, coordination, and delivery of the new program. You will bring proficiency in service development, leadership and client care that fosters a culture of best practice, innovation, and continuous improvement.

Through this unique opportunity, you will also aim to ensure that individuals living with life limiting illness receive high quality care and support tailored to their specific needs.

About you:

To be considered, you will have:
• Bachelor’s degree in Nursing, Social Work, Healthcare Management, Case Management or equivalent with current registration - Master’s degree preferred.
• Minimum 3 years’ post graduate experience in palliative care and aged care.
• Experience with service development, planning and implementation.
• Highly developed communication skills, including the ability to communicate effectively with a broad range of people.
• A warm and compassionate manner.
• A current driver’s licence and commitment to travel within Central Coast/Hunter Region and Sydney regions.
• A genuine alignment to BaptistCares’ purpose, mission and values.
