Housing Engagement Coordinator - Coraki

Based at Coraki, the purpose of this role is to provide effective assessment and planning to coordinate and support tenants and household members access to a range of individualised services to improve their wellbeing and achieve their goals in life.

Key duties and responsibilities:

  • Identifying the individual support needs, including the goals, strengths and vulnerabilities of Tenants and Household Members;
  • Developing support coordination  plans to coordinate access to Support Services for Tenants and Household Members;
  • Raising the awareness of Tenants and Household Members to Support Services, to facilitate greater utilisation of Support Services;
  • Following up on a periodic basis with Tenants and Household Members to assess and review Tenants’ and Household Members’ progress toward achieving their goals and the Target Outcomes; and
  • Adapting Tenant and Household Member support coordination plans to Support Services over time, as required.
  • Conduct a Tenant Needs Assessment/Reassessment for each tenant and related household member as required
  • Collect relevant data and regularly  report on outcomes achieved for tenants
  • Develop a Tenant Support Services Plan for each tenant and related household member, in a collaborative and client-focussed manner
  • Assist and support tenants to access services
  • Support tenants in self-management and relationships with carers and family members
  • Identify, facilitate and record referrals to appropriate support services for tenants and household members
  • Facilitate effective arrangements with government agencies and other service providers to provide support to tenants and household members

Essential qualifications, experience and competencies:

  • Certificate IV – Social Work or Community Services
  • Tenacity, tolerance, empathy and patience with tenants and their needs
  • Flexible and willing to work between locations as required
  • Demonstrated ability to facilitate the provision of effective and individualised support to achieve positive outcomes for tenants and household members
  • Experience working with high need, complex clients
  • High level negotiation skills and ability to liaise with key stakeholders
  • Competence in service assessment and documentation (including service planning).
  • NSW Driver’s Licence and NSW Working with Children Check for paid employment purposes
  • Previous experience working in community services or related discipline
