About the role
We are seeking an experienced Clinical Nurse Specialist to provide advanced clinical support, assessment and leadership within our residential aged care team at The Gracewood. In this role, you will use your clinical expertise to guide high‑quality care for residents with complex needs, including palliative care, dementia care, wound management, diabetes management and mental health.
You will work closely with Care Team Managers and Registered Nurses to conduct clinical reviews, develop and oversee care plans and mentor the team to ensure safe, person‑centred care. You will also contribute to continuous improvement initiatives, clinical governance and the development of clinical capability across the home.
You will help create a workplace where residents feel genuinely cared for and staff feel supported, working together with professionalism, integrity and a shared commitment to exceptional care.

About you
To be successful in this role, you will bring:
• Current registration as a registered nurse with AHPRA
• A postgraduate qualification relevant to aged care (for example geriatrics, dementia or palliative care)
• Experience working in aged care, including at least 12 months in your clinical specialty
• Demonstrated clinical leadership and the ability to mentor and support nursing teams
• Strong assessment, care planning and communication skills
• Knowledge of the aged care quality and safety standards
• A commitment to delivering care that reflects professionalism, compassion and a strong sense of responsibility
