As a Care Facilitator (Case Manager) servicing clients in the Far North Coast area, you will assist customers and their families to engage in services, enabling them to remain living in their home. You will empower our customers through individualised care planning and goal setting focused on our well-living approach.
This is a fixed-term full-time maternity coverage position commencing 12 June 2023 to August 2023.
